• Signed Prescription from your referring physician
• Medical Insurance Card
• Photo I.D.
• Precert/Referral (if necessary)

Try to arrive at our facility 15 minutes prior to your study unless specified otherwise. There will be some forms that you will need to complete prior to your exam.
